Sahyadri Foundation actively contributes to accelerating digital transformation to achieve UHC in LMICs by leveraging grassroots networks and technology-driven health interventions. We empower ASHAs, Anganwadi workers, and community volunteers through digital tools for health screening, awareness, and real-time data collection in rural and underserved areas of Maharashtra, India. Our digital outreach includes mobile-based health education, telemedicine linkages, and app-based beneficiary tracking to bridge the last-mile healthcare gap. We integrate with government systems and collaborate with tech partners to scale access to maternal-child health, NCD screening, and geriatric care. By combining local trust, trained human capital, and scalable tech platforms, we support early detection, preventive care, and timely referrals—cornerstones of UHC. As a field-based organization with strong digital adoption, we stand as a capable partner in co-creating inclusive digital health ecosystems that can be replicated across LMICs by 2030.
